The MLTC Program works in conjunction with insurance companies to prevent hospitalizations and create a better quality of life for members by educating them on healthy living. This is achieved by:

 

•          Initial Assessments Once someone joins the program a nurse comes to the home to conduct an initial assessment to establish the member’s health status and make any indicated recommendations or necessary interventions.
   

•          Semi-Annual Assessments Every six months the member will have a nurse visit the home to “check in” and assess the member’s health and progress
